Learning from Failures: Common AI Agent Pitfalls in BI

Last year, I watched our team's first AI agent deployment fail spectacularly. We'd spent months building an agent to automate report generation, tested it thoroughly in our sandbox environment, and proudly rolled it out to stakeholders. Within three days, it was disabled. The agent was generating technically correct but contextually meaningless reports, frustrating users and eroding trust in our entire BI initiative.
