Use cover/skip/merging instead of track_files/add_filter/use_merging,
and clear previous resultsets at the start of `rake coverage` so that
results from an earlier run within the merge timeout are not mixed in.

Remove coverage.json and lcov.info at the start of `rake coverage` so a
failed run cannot leave a previous report looking current, and leave the
lock file alone since unlinking it breaks flock for concurrent runs.
