Petition: Fund and Improve Mental Health Service transition into the Community

Fund an improved mental health service enabling a seamless transition from acute crisis care (Home Treatment/Rapid Response) and community support. Establish a statutory "Continuity of Care" guarantee with a safety/care plan before a patient is discharged from intensive services.
